Registration & Medical
Before taking the field, all players must have the following documents signed and submitted to their respective team managers:
- Player Registration Packet (PDF) - Includes emergency contacts, club expectations, and liability waivers.
- Medical Release Form (PDF) - Required for all travel tournaments. Must be notarized.
- Concussion Information Sheet (PDF) - State-mandated educational material on head injuries.
Financial Assistance
Southwest Soccer Club believes financial hardship should never keep a talented player off the pitch. We offer limited scholarships each season based on need.
- Financial Aid Application (PDF) - Must be submitted before the start of the Fall season.
- Sponsorship Deck (PDF) - For local businesses interested in supporting our teams.
